Polygenic Risk Score testing for cardiovascular disease

Your provider has offered the opportunity to obtain a score which assesses your genetic risk of heart disease.

Please enter your contact information and Provider ID (given to you by your Emory physician), and we will mail you a kit to perform the test. Once you return the kit with a saliva sample, we will process the information and deliver results to your provider in 4-5 weeks. Your provider will be able to review your test results with you at your next scheduled visit.

We charge $199 for this testing service, which is a discounted rate offered exclusively to patients of the Emory Center for Heart Disease Prevention.
